This Disclosure Agreement is made and entered into on the date of submission by and between Parental IMPACT LLC, with its principal place of business located at 1300 West Sam Houston Pkwy S, St 500 #444 Houston, TX 77042 and Contributor/Customer (individual submitting this agreement or representative), with an address at located on submission. WHEREAS, Parental IMPACT publishes a magazine Parental IMPACT which includes various features and advertisements; and WHEREAS, Contributor/Customer desires to submit information, including but not limited to text, images, and advertisements, to Parental IMPACT for consideration in the Magazine; and WHEREAS, Parental IMPACT has the authority to select which stories, images, and advertisements will be included in the Magazine, in which issue they will appear over the course of the year but guaranteed to be featured in one of the 4 issues, and may alter or change the submitted information as necessary; NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties agree as follows: Authority to Select Stories, Images, and Advertisements. Parental IMPACT has the sole and exclusive authority to select which stories, images, and advertisements will be included in the Magazine. Authority to Choose Magazine Issue. Parental IMPACT has the authority to choose which issue of the Magazine to include the Contributor's/Customer's story, images, or advertisement over the course of the year but guarantees inclusion in one of the 4 issues. Alterations and Changes. Parental IMPACT may alter or change the submitted information as necessary to fit within the Magazine format or for other reasons at its sole discretion. Ownership and Originality. Contributor/Customer represents and warrants that it is the sole owner of the submitted information and that all images and stories have not previously been used or submitted to another magazine or publication. Payments. Contributors agree to the terms of non-compensation until further notice. Written communication will be made available at the time of any changes. Customers agree to advertisement and Parental IMPACT's rates as stated in media kit and advertisement requirements. Confidentiality. Contributor/Customer agrees to keep confidential all information related to Parental IMPACT's selection process and the content of the Magazine. Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the State of [state/country]. IN WITNESS WHEREOF, the parties have executed this Agreement as of the date of submission. By submitting, I agree to the above terms and conditions.

By submitting this form, I certify that I have the rights and permissions to use these images/photos and take responsibility for their use in the interview. I will avoid sending the same article/story and images/photos to different magazines during the time Parental IMPACT publishes my article/story. I hereby grant Parental IMPACT Magazine a non-exclusive license to use the photograph in its submitted form, subject to re-sizing to fit the magazine format, for publication in Parental IMPACT Magazine or any other information pertaining to Parental IMPACT Magazine. By submitting, I agree to the above terms and conditions.

I hereby authorize Parental IMPACT, its affiliates, and its agents to use and publish the photographs taken of your child/children (if applicable) for use in Parental IMPACT Magazine and related promotional materials. I will avoid sending the same article/story and images/photos to different magazines during the time Parental IMPACT publishes my article/story. I understand and agree that the Photos may be edited, altered, or modified in the Company's sole discretion. I also understand that Parental IMPACT has no obligation to use the Photos. I hereby release and discharge the Parental IMPACT from any and all claims and demands arising out of or in connection with the use of the Photos, including any claims for invasion of privacy, defamation, or copyright infringement. I also agree to indemnify and hold harmless Parental IMPACT from any and all claims, damages, or expenses arising out of or in connection with the use of the Photos. I represent and warrant that I am the parent or legal guardian of any child depicted in the Photos, and that I have the authority to grant the rights set forth in this Photo Release Form. I have read and fully understand this Photo Release Form and its contents. I acknowledge that I have received good and valuable consideration for the rights granted herein. By submitting, I agree to the above terms and conditions.

I declare that I am the author/creator and owner of the information submitted for publication in Parental IMPACT Magazine. I hereby grant Parental IMPACT Magazine a non-exclusive license to publish, reproduce, distribute, display, and otherwise use the content in any format or media now known or developed in the future, including but not limited to print, online, and digital formats, for the purpose of publication in Parental IMPACT Magazine.

 I understand that Parental IMPACT will retain copyright ownership of the content and will credit me as the author/creator in all publications of the content. I also understand that I may not use the content for non-commercial purposes, such as personal portfolio or promotion until the magazine has been published and with the understanding that Parental IMPACT Magazine will be credited as the original publisher.

 I represent and warrant that the information is original and does not infringe upon any copyright, trademark, or other proprietary right of any third party. I agree to indemnify and hold Parental IMPACT Magazine harmless from any and all claims, damages, and expenses arising out of or in connection with any breach of this representation and warranty.

 I understand that Parental IMPACT Magazine may edit or revise the information for grammar, punctuation, spelling, style, format, or may materially alter the content without my prior approval. Once submitted, I approve of any edits or revisions made by Parental IMPACT Magazine, I may not withdraw my submission from publication.

 This agreement constitutes the entire understanding between Parental IMPACT Magazine and the individual submitting regarding the publication of the content, and supersedes all prior negotiations, understandings, and agreements, whether written or oral.

 I have read this agreement and understand its terms and conditions. By submitting, I agree to be bound by the terms of this agreement.

This Non-Refundable Payment Agreement is made and entered into as of date of submission, by and between Parental IMPACT and the individual submitting the agreement. WHEREAS, the Author has submitted a magazine article (the "Work") to be published by the Publisher; WHEREAS, the parties have agreed that the Publisher will pay the Author a fee for the publication of the Work; NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties agree as follows: Non-Refundable Payment. The Publisher agrees to pay the Author the sum of agreed payment amount as full compensation for the publication of the Work. The payment shall be made upon submission of the Work and shall be non-refundable. Grant of Rights. The Author hereby grants to the Publisher the non-exclusive right to publish the Work in the magazine as specified by the Publisher. The Author represents and warrants that the Work is original and that he/she has the right to grant such rights to the Publisher. No Other Compensation. The Author acknowledges and agrees that the payment provided for in this Agreement shall be the sole compensation due to the Author for the publication of the Work. Confidentiality. The parties agree to keep the terms and conditions of this Agreement confidential and not to disclose them to any third party, except as required by law. Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the state in which the Publisher is located. Entire Agreement. This Agreement constitutes the entire agreement between the parties and supersedes all prior negotiations, understandings, and agreements between the parties. IN WITNESS WHEREOF, the parties have executed this Agreement as of the date submitted. By submitting, I agree to the above terms and conditions.
